Before submitting a request to use Friends Church for any event the person or group requesting the use must read, acknowledge and agree to abide by Friends Church Facility Use Policy.

After examining the Facility Use Policy, answer the following questions and fill in all the required information below that applies. The last three questions only need be supplied if applicant is planning to use our facilities for a wedding.
